Prior to submitting a full proposal, please click on the 2018 Grant Application link below, and when prompted, click on the Guest Account link to upload your Initial Inquiry letter and 501(c)(3) certification.
(In your inquiry letter, please tell us briefly about the organization, the purpose of the grant, and if you have an amount in mind, please include it in the letter).
The Dispensing Committee will review the inquiry letter and a member of our staff will contact the applicant electronically with either an invitation to submit a full proposal (by providing the applicant with an Account Name and Password), or a letter explaining why the proposal could not be funded. This review process may take several weeks to complete.
If you are invited to submit a full grant proposal, and have been assigned an Account Name and Password, please click on the Returning Applicants link below to complete a Donation Request form and upload additional information; such as the organization's most recent audit, a one page budget of the project, and any additional information that may further describe the request.
